Infliximab, eternecept and adalimumab are tumour necrosis alpha factor (TNFalpha) antagonists which recently became part of several auto-immune diseases therapeutic arsenal, mainly in rheumatoid arthritis, ankylosing spondilitis, psoriatic arthritis, as well as inflammatory bowel diseases. TNF-alpha plays a crucial role in many aspects of immune system development, immune-response regulation and T-cell-mediated tissue injury. It is a cytokine with both pro-inflammatory and immunoregulatory properties and is involved in normal inflammation and immune responses. TNF-alpha is an important growth factor for prothymocytes and thimocytes, and thereby influences the generation of the T-cell repertoire. In the peripheral immune system, TNF-alpha participates in antigen-presenting cellfunction and in regulating apoptosis of potentially auto-reactive Tcells. So that, immunological complications with drugs that antagonize TNF-alpha receptors were expected to occur. Demyelinating peripheral motor neuropathy is one of them, but it is relatively uncommon. We report a patient who had a severe form of Crohn’s disease, who developed a peripheral neuropathy with multiple motor conduction blocks at atypical sites, with axonal damage signs, probably associated with infliximab treatment.

A case of multifocal-motor-neuropathy-like disease with conduction blocks under infliximab with spontaneous progressive recovery.

Treatments with anti-TNF alpha (ATA) are now commonly used in certain inflammatory rheumatic diseases such as spondyloarthritis, rheumatoid arthritis or psoriatic arthritis, and inflammatory bowel disease (IBD). Among the different therapeutic classes available, there are, firstly, chimeric or humanized monoclonal antibody such as infliximab (Remicade) and adalimumab (Humira) and, secondly, eta...

A variant of multifocal motor neuropathy with acute, generalised presentation and persistent conduction blocks.

OBJECTIVE Multifocal motor neuropathy with persistent conduction blocks is classically described as a chronic neuropathy with progressive onset, and acute forms have not previously been characterised. We report four cases of severe motor impairment with acute and generalised onset and with persistent motor conduction blocks. PATIENTS AND RESULTS An acute tetraparesis with diffuse areflexia bu...

Ultrasonography of MADSAM neuropathy: focal nerve enlargements at sites of existing and resolved conduction blocks.

Using the emerging technique of peripheral nerve ultrasonography, multiple focal nerve swellings corresponding to sites of existing conduction blocks have been described in demyelinating polyneuropathies. We report two cases of multifocal acquired demyelinating sensory and motor neuropathy (MADSAM). In the first, multiple focal nerve enlargements were detected by ultrasound at sites of previous...

Multifocal motor neuropathy.

We report the case of a patient with multifocal motor neuropathy. Electrophysiological studies showed typical multifocal conduction blocks, but the search for anti-GM1 antibodies was negative. This case provide the heterogeneity of this clinical entity.

TNF-Alpha in Peripheral Neuropathy Patients with Impaired Glucose Regulation

Impaired glucose regulation (IGR) is the prestate of diabetes; about 1/3 of IGR patients will develop to diabetes finally. In this study, we investigated the serum tumor necrosis factor-alpha (TNF-α) and interleukin-6 (IL-6) levels in peripheral neuropathy impaired patients with impaired glucose regulation (IGR). A total of 70 IGR patients received the conventional nerve conduction test, includ...
